COWS

Clinical Opiate Withdrawal Scale

An eleven-item clinician-rated scale for opioid withdrawal, scored 0–48. Item ranges vary. Often used to time buprenorphine induction (commonly a score of ≥8–12 with objective signs).

For use by qualified clinicians as a decision-support aid. Scores support but do not replace clinical judgment. This is not a diagnostic instrument and no information is stored or transmitted.

Rate each of the eleven signs and symptoms based on observation and the patient’s report at the time of assessment.

  1. 1. Resting pulse rate

    Measured after the patient is sitting or lying for one minute.

  2. 2. Sweating

    Over past ½ hour, not accounted for by room temperature or activity.

  3. 3. Restlessness

    Observed during assessment.

  4. 4. Pupil size
  5. 5. Bone or joint aches

    If the patient had pain previously, only the additional withdrawal component is scored.

  6. 6. Runny nose or tearing

    Not accounted for by cold symptoms or allergies.

  7. 7. GI upset

    Over last ½ hour.

  8. 8. Tremor

    Observation of outstretched hands.

  9. 9. Yawning

    Observed during assessment.

  10. 10. Anxiety or irritability
  11. 11. Gooseflesh skin
